Returning the Equipment

Objective

Return your HMS Shirt at your 12-month follow-up visit

Procedure #1 in Clinic

1. Contact your Research Team
2. Schedule and confirm the date of your 12-month follow-up visit
3. Prepare your HMS equipment (see Equipment Checklist below)
4. Bring your HMS equipment to your visit
5. Return the equipment to the Research Team

Procedure #2 Home Pick-up

If you are not able to bring the equipment to your clinic visit, you can arrange for home pick-up

1. Contact your Research Team
2. Schedule a date for home pick-up
3. Place all the Equipment in the original boxes (refer to the Equipment Checklist above)
4. Seal the box with tape
5. Wait for the Shipping Provider to pick up the box
6. If you need assistance, contact your Research Team
